Activcore is an out-of-network provider of physical therapy. Our prices range from $160 to $250 per hour session depending on your specific provider and the type of service rendered. Most people only need a handful of sessions to feel better and get on a path to healing. We also have fitness options available at lower pricing.
Our policy is to collect payment in full at the time of service. After each session, we will submit an insurance claim so that you can get reimbursed directly from your insurance company. Our billing specialists are available to verify and explain your insurance coverage.
Why are we out-of-network?
In a word, freedom. With the ever-changing insurance landscape, physical therapists are being forced to juggle multiple patients at the same time, typically pushing you off to an assistant or aide to do cookie cutter exercises that could have been done at home. At Activcore we refuse to play this volume game. Working outside of the insurance networks allows us to treat one person at a time, prioritizing your health over everything else for the fastest results possible.
What does it mean to go out-of-network?
Out-of-network simply means there's no financial contract between Activcore and the insurance company. However, this doesn't mean the service isn’t covered by your insurance company. Our billing specialists can verify if you qualify for reimbursement.

You should expect to start receiving reimbursement checks within 4-6 weeks. If you have a yearly deductible, that amount must be met before any reimbursement is issued.

Can I use a Health Saving (HSA) Card?
Yes, our physical therapy and fitness services are typically covered by Health Savings Accounts (HSA) and Flexible Spending Accounts (FSA).
Am I able to get a Good Faith Estimate (GFE) of the anticipated charges?
At Activcore, we believe that price transparency should be an essential part of the treatment experience. Therefore, upon your request, we can put together a Good Faith Estimate in an effort to avoid any billing surprises. This is an estimate of the total anticipated charges during your course of care. It is based on the costs of services that are reasonably expected for your healthcare needs at the time the Good Faith Estimate is created. It is just an estimate and not the final overall charges. It does not include any unexpected or unknown costs that may arise during your course of care. Your provider may recommend additional services that are not reflected in the Good Faith Estimate. In other words, the final cost could exceed the estimate. In the event you are billed more than what was expected, you have the right to dispute the charges, negotiate the bill, and/or ask us if any financial assistance or payment plans are available. If you are not satisfied with our billing practices, and the final overall cost is more than $400 over the amount shown in the Good Faith Estimate, you are entitled to start a dispute resolution with the U.S. Department of Health and Human Services (HHS) within 120 days of the date of the original bill.
